The bilingual Community Health Worker Specialist position is within the Childhood Lead Poisoning Prevention Program which provides case management, information, and education to families and children with elevated blood lead levels and coordinates Environmental Health inspection and abatement in the community.
Under the guidance of the Senior Public Health Nurse and Supervising Public Health Nurse, the CHW Specialist in this program is responsible for:
- Providing care coordination, education, and outreach to families and children with elevated blood levels
- Making phone calls to families to provide education at the direction of the Public Health Nurse
- Providing written instructions and education
- Mailing letters and brochures
- Entering information in Lead Program Tracker
- Making case notes in Response and Surveillance System for Childhood Lead Exposures (RASSCLE II) , the State of California’s web-based statewide childhood lead poisoning case management and reporting system
- Working in close coordination with the Public Health Nurses in CLPPP
- Performing outreach activities and making presentation in public spaces as needed
- Knowledge and education in community health work
- Knowledge of basic public health principals, social determinants of health and understanding of health and social disparities
- Previous training and/or experience providing health information and education to diverse populations
- Intermediate Microsoft Office skills
- The ability to work within databases and accurately document outreach and education activities
- The ability to research questions, identify resources, enter data, problem solve, and document case notes
- Presenting/public speaking skills are desirable
- A Community Health Worker Certificate is desirable

Minimum Qualifications
Knowledge, Skills, and Abilities
Considerable knowledge of: Department of Health Services' programs, policies and procedures; a wide variety of community health and social services resources; psycho/social/cultural issues faced by members of the underserved and at risk communities served; a wide variety of public and private health care community support services.
Working knowledge of: Methods and procedures of training staff and volunteers; methods and procedures of health education; principles and practices related to public speaking and the development of formal presentations; methods and techniques of conducting interviews; counseling techniques.
Knowledge of: basic public health principles, health behavior theories, healthy lifestyles and self-care, as well as the social determinants of health and related disparities.
Ability to: conduct self-management coaching, manage conflict, use interviewing techniques, coordinate care, communicate with residents of the underserved and at risk communities and with representatives of public and private agencies providing community health-related services; work effectively and gain support with clients of different ages and backgrounds; provide training and education to staff, clients, community agency staff, and the public; communicate effectively both orally and in writing in English; work well with others; function as a team member, and benefit from supervision and training; carry out a variety of assignments and balance priorities in assigned area(s) with minimal supervision.

The selection procedure will consist of the following examination:
An Application & Supplemental Questionnaire Appraisal Examination (weight 100%) will be conducted to evaluate each applicant's application and supplemental questionnaire for satisfaction of minimum qualifications (pass/not pass); and for educational coursework, training, experience, knowledge, and abilities which relate to this position. Each applicant will be evaluated based on the following criteria:
- Relevance of work history, related experience, and achieved level of education and/or training as described in the application and responses to the supplemental questions.
Candidates demonstrating possession of the minimum qualifications will be placed on an employment list in order of most qualified to least qualified based on the achieved score received in the Application & Supplemental Questionnaire Appraisal Examination which evaluates the education, training, experience, skills, knowledge, abilities, and other qualifications that you demonstrate in your application and/or response to the supplemental questionnaire. Please be aware that resumes and cover letters are not typically evaluated in the Application & Supplemental Questionnaire Appraisal Examination. Scores may be adjusted based on such factors as the number of candidates, anticipated vacancies, past practice, and natural breaks in the scores achieved by this group of candidates.
